The Chaplaincy Care Seminar includes experiential practice on grief, trauma, moral injury, and crisis response. Please read and confirm the following. Items marked ✦ are required to participate.

Voluntary participation ✦

I am participating voluntarily. I understand I may pause, step out, or opt out of any exercise at any time without penalty, and I will be met with grace when I do.

Confidentiality covenant ✦

What is shared by other participants stays in the room. I will not share names, identifying details, or stories outside the cohort. I understand facilitators are bound by mandated-reporting laws when there is imminent risk of harm to self, others, a child, or a dependent adult.

Emotionally intensive content ✦

I understand the seminar addresses grief, trauma, suicide risk, and moral injury. On-call clinical and pastoral support is available throughout. I will notify a facilitator if I need private support.

Photo / media release (optional)

I permit the Academy to use general group photos or video from teaching sessions for educational and promotional use. Close-ups and small-group practice moments are never photographed.

Anonymous research use (optional)

I permit my anonymized pre- and post-seminar survey responses to be used in the Chaplaincy Care Needs Assessment (CCNA-HI) research project. No identifying information is ever published.

Employer authorization (if attending on work time)
